I'll second the cut2medesigns. I am a very big guy who has always just had to make due with what I could scrounge together. They got me great pieces that fit my body wonderfully. They taylor it to you (sizes, measurements) specifically, and they have no problem with you sending it back for mending to fix something. Us big guys always have that one weird quirk in our bodies, and they can help accomodate that. I'm about to buy my second round of gear from them.
Also, once they have your measurements, they can suggest other designs that properly show off as much of you body as you desire. They worked with me and I now have three items that show off a big beautiful body and I feel very good about.
Competitive wrestlers come in pretty big sizes; if you're looking for something with a fetish aspect (like a string singlet, or something) you might have to find a specialized vendor, but you should be able to find a plain old freestyle singlet at any number of online wrestling supply retailers.
